Documents to be submitted with this application:
☐ Written documentation from Dojo Instructor of applicant’s regular class attendance. (Synopsis or
copies attendance records)
☐ A written essay (1000 words) explaining why applicant should receive this scholarship. The items
listed in the requirements section are topic suggestions only.
☐ Written recommendation from applicant’s Dojo Instructor
☐ An unedited video of the applicant performing Gekisai Sai Di Ni and the kata required for
present rank. Videos should be sent by email.
☐ Proof of intent to attend the IOGKF event sponsored by the Growth & Development Fund
(i.e. copy of Airline Ticket, Hotel Reservation, Event Registration)

Entries must be received, in full, by deadline date. No late or incomplete entries will be considered.
In the event that the scholarship is awarded and the recipient finds they are unable to attend the event, they agree to
notify the Growth & Development committee immediately. The funds will then be awarded to the next most qualified
candidate.
A new application must be submitted for every Growth & Development Fund sponsored event
